Cancellations and Fees
Cancellations:
Two full business day notice is required to cancel an appointment. Appointments cancelled with less than 1 business day will be charged $100. You must send an email or call and leave a voicemail.
For Example: An appointment Tuesday thru Friday: for an 11:00 appointment on Tuesday, cancellation notification would need to occur prior to 11:00 Monday. Monday appointments: cancellation would need to occur prior to the scheduled appointment time on the Friday prior to the visit

Payment Types Accepted:

The current self-pay rates for DRS providers are as follows:
- Initial Diagnostic Evaluation (physician) [60 to 90 min.] – $350
- Medication management (physician) [30 to 45 min.] – $250
- MAT Intake – $400
- Psychiatric Telephone Consultation – $10/min. [capped at $200/30min.]
Please Note: Fees for evaluations and specialty services may with discretion of provider if more time and / or ancillary services are required.
Other Fees:
- Form / Document Completion [outside of an appointment] – $150
- Certain complex documents (SSDI forms, for example) will be billed at $200 / hour
- Health Record Requests – $25 [for first 20 pages] + $0.50 [for each page beyond 20]
- Prescription Refill [outside of an appointment – $50
- Returned check fee: $50
Credit card fees: due to the rising cost of credit card processing, effective January 1, 2025, a fee of 2.9% + $0.30 will be added to all credit card transactions. If you would prefer to avoid this fee, please contact our office in advance to arrange for alternative payment.
Credit card dispute fee: $50. 00 Disputed charges may result in discharge from care. Do avoid this, we recommend contacting our office if you have a concern about your account prior to contacting your credit card.
