Individual Outpatient Services
DRS seeks to address clients’ personal needs in an outpatient utilizing an in-depth holistic approach also empowering clients with psychoeducation. A conservative medication approach in tandem with therapy is usually taken. In addition, coordination with other providers is important so that there is continuity of care allowing for less duplication of similar medications, less medication interactions, therefore potentially less foreseeable adverse outcomes.
This is an outpatient service and is not equipped to handle emergencies / acute medical, psychiatric and substance issues. In acute issues seek higher care facilities. Clients are encouraged to also engage the care of PCP in maintenance care ongoing as well as recommended ancillary psychotherapeutic care.
Services Include:
Individual Intake Evaluation
Then when delving into the client’s presenting chief complaints, history, and identifying their aspirations, we can tailor a personalized plan that addresses their unique needs. This also allows me to see what their expectations for treatment are, what factors are blocking their achievement thereof and how to [plan treatment for them, as well as some elements of mental status such as insight and judgement that will require therapy adjunctively in their treatment. Addressing medical issues and how the client is attending to self-care is also discussed as part of an integrated approach as this contributes to overall psychological wellness. Medications are discussed as is the time they are taken and their duration of action, their impact on sleep, factors affecting bioavailability, their interactions with all the other medications the client is on, as well as other things such as weight concerns, sexual side effects, etc. The client and I will discuss their goals in terms of medication, their life in general, and what is getting in the way of their success.
Medication Management
These visits are about 30 to 45 minutes depending on the complexity of the clients’ issues. In cases that involve MAT as in Dual Diagnosis, these must be scheduled as 45-minute follow-up sessions due to the sum of DEA requirements such as PMP documentation and the need for multiple issues that need to be discussed. There are other types of cases that are better addressed with a longer appointment. This provider is genuinely concerned with a client’s wellbeing so would like to do more than write prescription… Rather would expect to hear about such things as: interval history, health issues, current stressors, medications change from other providers, etc. Therapeutic techniques such as CBT, RET, Behavioral, Psychodynamic, and other individualized supportive recovery techniques are employed… The frequency of these visits varies but must meet the clients’ needs. Dr Deerfield goes out of her way to titrate medications safely and provides superior psychoeducation.
